Appointment & Cancellation Policy
Any consultation fee (if applicable) is due at booking.
Cancellations require 24-hour advance notice.
No-shows or late cancellations may result in forfeiting the fee and additional charges.
Late arrivals may be rescheduled if more than 15 minutes past the appointment time.
Full payment is required at the time of service. Accepted methods: credit/debit cards, cash. We do not bill insurance unless otherwise stated. Any consultation fee ( if applicable) is non-refundable. Services rendered and medications dispensed are not refundable.

Terms of Use
Prescriptions are issued only after clinical evaluation. We do not prescribe controlled substances without necessary protocols and follow-up. Please request refills at least 5 business days in advance. Compounded/specialty medications are non-returnable and non-refundable.
Labs may be required for treatment planning and compliance. You are responsible for timely completion of labs. Results are reviewed and discussed during your follow-up. Labs may be billed separately by third-party providers.

Card on File Policy
To provide efficient and transparent billing, S&T Urgent Care requires a card on file for all patients receiving care, whether using insurance or paying out of pocket. This policy ensures prompt payment for services and reduces billing delays for both insured and self-pay patients.
🔹 How It Works (For Insurance Patients):In addition to collecting any required co-pays at the time of service, patients using insurance must keep a credit or debit card on file. After your visit, we submit a claim to your insurance provider. Once processed, you and S&T Urgent Care will receive an Explanation of Benefits (EOB) showing any remaining patient responsibility. If a balance is due, we will notify you by email, and your card on file will be charged 10 calendar days later, unless you contact us to make other arrangements. If your insurance covers the visit in full, no charge will be made to your card.
🔹 How It Works (For Cash-Pay / Non-Insurance Patients):Patients not using insurance (cash-pay) are expected to pay in full at the time of service. If for any reason the full balance is not paid during the visit, the remaining balance will be charged to the card on file within 10 calendar days. This ensures uninterrupted access to care and minimizes manual billing follow-up.Exemptions:The Card on File policy does not apply to:Patients experiencing a medical emergencyPatients covered under Medicare or Medi-CalPatients being seen for Occupational Medicine or Workers’ Compensation servicesAll cards are securely stored and processed in compliance with HIPAA and PCI-DSS standards.
